Home Health Manager

Join WelbeHealth in Rosemead, CA as a Home Health Manager, a pivotal role in our mission to support seniors in their homes and communities. You will manage comprehensive medical care and community-based services, specifically overseeing skilled home healthcare and ensuring high-quality care delivery in nursing homes and board and care facilities. This role requires a strong leader to direct a high-performing home care department, ensuring strict adherence to workflows and policies. You will guide and develop the home care staff through hiring, training, and coaching, while assessing the effectiveness of nursing assessments and interdisciplinary team participation. Your expertise will ensure home care nurses provide and meticulously document nursing interventions according to participant care plans.

Responsibilities include:

• Directing a high-performing home care department, ensuring workflows and policies are followed.
• Guiding and maintaining home care staff, including hiring, training, and coaching.
• Assessing the effectiveness of home health nursing assessments and interdisciplinary team (IDT) participation.
• Ensuring home care nurses deliver and document nursing interventions as agreed upon in participants’ care plans.
• Overseeing the quality of care provided to participants in nursing homes and board and care facilities.

Qualifications:

• California Registered Nurse (RN) License is mandatory.
• A Bachelor's Degree is preferred; additional years of relevant experience may be substituted.
• A minimum of two (2) years of nursing experience in a home health setting, preferably with a frail or elderly population.
• Proficiency in nursing practices necessary to treat frail, elderly participants and manage complex clinical scenarios.
