Mercy Health by Compassus, a leading healthcare organization in Youngstown, OH, is seeking a motivated Hospice Admissions Coordinator to join our team. This vital role is responsible for the seamless processing of patient referrals, efficient coordination of hospice admissions, and ensuring strict adherence to all hospice regulations and requirements. The Hospice Admissions Coordinator will play a key part in fostering positive and collaborative relationships with internal team members and external healthcare partners, physicians, and community stakeholders.
Key Responsibilities:
- Participate actively in staff meetings, educational programs, and community outreach events as required.
- Identify and communicate patient needs and potential solutions to the supervisor, contributing to process improvement.
- Receive, review, and process patient referrals from physicians, patients, families, friends, and community sources.
- Collaborate with physicians to obtain necessary admission orders. For non-RN Admissions Coordinators, ensure a Registered Nurse is present for verbal orders or facilitate the faxing of physician orders.
- Initiate and manage pre-certification authorizations with insurance companies.
- Schedule appointments for informed consent discussions and RN assessment visits with patients and families.
- Clearly explain hospice services, benefits, and care philosophies to patients, family members, and significant others.
- Communicate appointment schedules to appropriate interdisciplinary team members.
- Ensure the completion of the admission process in strict accordance with agency and corporate policies.
- Facilitate timely notification of admissions to the assigned Interdisciplinary Team (IDT) and all relevant ancillary providers.
- Accurately enter admission data into the designated third-party system.
- Meticulously maintain patient charts, ensuring all documentation is complete, signed, and dated before submitting to the Team Coordinator.
- Actively engage in marketing efforts to promote referral sources and hospice services.
- Build and maintain relationships with long-term care staff, hospital personnel, and physicians to champion hospice care.
- Provide essential back-up support for the Hospice Care Consultant as needed.
- Document all marketing contacts and activities as directed by management.
- Perform other duties and responsibilities as assigned to support the team and organizational goals.
